Before we start, I want to get to know a little bit about each of you, so that I can help you make the most of this class.

Please answer each of the questions so that I can better understand what tools you have available, your academic goals, history, and any information you feel is relevant. This is not graded, but it is required.

This information is purely for me to get to know you better. It is private and I will not share or disclose it with anyone.
