ABOUT THE QUESTIONNAIRE
This detailed Questionnaire provides me with important information to help me communicate with you, support your learning needs, and work with you to create a positive learning experience. All answers are kept strictly confidential (with the exception of your name, primary phone # and email if you grant me permission to put that information in a class contact list.)
If you feel uncomfortable answering any question, I totally understand. Just leave the question blank or insert a note like "I'd prefer not to answer."
If any information you include in the Questionnaire changes for you as the semester progresses, you can view your Questionnaire at any time under "Grades" and record changes in the comments field to the side of the assignment. (Just send me a message so I know if you do that.)
INSTRUCTIONS
Click the blue button that says, "Start Assignment." Select Text Entry submission option. Highlight and copy all the questions, including the spaces between them, and paste directly into your text entry box. Then, type your answers directly after each question as you read the chapter. Keep an eye on the question #s, as Canvas sometimes likes to re-number the questions for no reason; if that happens, just change the number to the correct number. When done, click "Submit Assignment."
NOTE: I have also included a PDF of the Questionnaire below if you'd prefer to type or handwrite your answers. If you use the PDF, please save/rename or scan your completed Questionnaire and upload the saved/scanned document to Canvas under this assignment.
