Objective:
Apply your knowledge of nutrition care by writing an NCP (Nutrition Care Process) in ADIME format, including a clear PES (Problem, Etiology, Signs/Symptoms) statement.

Case Description:
Meet Mrs. Smith. She is a 52-year-old woman, 5'2", and weighs 150 lbs. She has been diagnosed with Irritable Bowel Syndrome (IBS), with symptoms that alternate between constipation and diarrhea. She experiences stomach cramps and notes her symptoms worsen when she’s anxious—especially before giving weekly presentations at work.

Here is what she typically eats:

• Breakfast: Coffee with cream, Cheerios with milk and fruit

• Snack: Granola bar or candy bar

• Lunch: Sub sandwich, chips, soda

• Dinner: Meat, potatoes or rice, vegetables

• Evening snack: ~1 cup of ice cream

Mrs. Smith does not engage in regular exercise.
Her lab values show:

• Slightly elevated LDL (140 mg/dL)

• Slightly low HDL (45 mg/dL)

• All other labs are within normal limits

Instructions:

1. Write an NCP for Mrs. Smith in ADIME format.

   • A: Assessment (Include relevant dietary, medical, and lifestyle information - A, B, C, D's)

   • D: Diagnosis (Write one PES statement that addresses her key nutrition problem)

   • I: Intervention (Include clear, specific dietary recommendations based on her diagnosis and preferences)

   • M/E: Monitoring and Evaluation (How will you measure progress or outcomes?)

2. Make sure your dietary advice is practical and realistic.

   • Suggest foods that may help with her IBS symptoms

   • Identify foods or patterns that may worsen symptoms

   • Consider her cholesterol levels and lifestyle in your recommendations

Demonstrate your ability to apply clinical nutrition knowledge in a real-world context. Your recommendations should be evidence-based, personalized, and easy for Mrs. Smith to follow.
