THAR 13.1: Student Questionnaire

Hello THAR 13.1 Folks: 

This detailed Questionnaire provides me with important information to help me communicate with you, support your learning needs, and work with you to create a positive learning experience.  All answers are kept strictly confidential (with the exception of your name, primary phone # and email if you grant me permission to put that information in a class contact list.) 

If you feel uncomfortable answering any question, especially those that are listed as "optional", I totally understand.  Just leave the question blank or insert a note like "I'd prefer not to answer."

If any information you include in the Questionnaire changes for you as the semester progresses, you can open up your Questionnaire again at any time and adjust them as needed.  (Just send me a message so I know if you do that.)
